- *Key Responsibilities:**
- - Lead and collaborate to provide management and delivery of patient care plans
- - Support patients in the state PA, and surrounding areas
- - Eligible for a $15,000 first-year success bonus to reward key achievements during the first year
- *Schedule and Shift Details:**
- - Monday to Friday, 8 am to 5 pm; on-call rotation only 1 weekend every 4-8 weeks, with on-call hours limited to Saturday & Sunday, 8 am to 5 pm, for SOC visits. No on-call during the week or after hours.
- *Qualifications:**
- - Associate degree or higher in Nursing
- - Minimum of 1 year of RN experience
- - 1 years of experience in Electronic Medical Record (EMR) or other EMR systems preferred
- - Valid Registered Nurse (RN) license in the state of Pennsylvania
- - Certification in Basic Life Support (BLS) by the American Heart Association
- CPR certification (if you don't have any of the top 2 certifications - we will help you get it!)
- - Valid Driver's License, Reliable Mode of Transportation, and Auto Insurance
